After we receive your Registration Form, Mari-Ann will contact you to schedule an appointment for your Personal Pre Body Composition Testing and Measurements and to complete your paperwork and Registration Fees for Boot Camp.

RELEASE

(This RELEASE part of registration is for your information only, it will be completed and signed at the pre-assessment evaluation - not online)

This release is entered into between the undersigned and The Nordic Warrior Goddess., its officers, affiliates, and executors in addition to the District of Osoyoos. The purpose of The Nordic Warrior Goddess is to provide fitness instruction and coaching for various levels of athletes/individuals.

The undersigned hereby acknowledge that the following was explained to me and/or agree to the following:

  1. Acknowledges that Mari-Ann Drevvatne is not a physician and is not trained in any way to provide medical diagnosis, medical treatment, or any other type of medical advice.
  2. Acknowledges that coaching/training is another tool for teaching athletes/individuals about themselves, and though The Nordic Warrior Goddess. guarantees results, the individual is completely responsible for setting REALISTIC goals, attending ALL events, seminars, tours, and classes that are required and is willing to follow ALL nutrition guidance set by The Nordic Warrior Goddess. We cannot guarantee results for participants with pre-existing medical conditions that either hinder weight loss, (Example: thyroid conditions) or require special medical supervision. (Example: Diabetes, High Blood Pressure, Kidney or Renal conditions, etc.)
  3. Acknowledges that the undersigned has been told if they feel tired, feel pain or feel out of the ordinary in any way either related to your training, or otherwise, that the undersigned should contact a physician at once.
  4. Acknowledges that boot camps, aerobic classes, medicine ball training, agility training, running, weight training, core strength training, obstacle courses, hiking, and any other related sports are an extreme test of one's mental and physical limits and carry with it potential for damage or loss of property, serious injury and death. That the undersigned assumes the risks of participating in these types of events/activities, that they are fit, and they have a regular medical physician they can contact regarding any medical problems that they might develop. The undersigned expressly waive, release, discharge and agree not to sue from any liability of death, disability, personal injury, or action of any kind The Nordic Warrior Goddess. for the undersigned participating in said sporting events and/or training for said sporting events.

The Undersigned agrees that this is the full agreement between the parties, that The Nordic Warrior Goddess. nor anyone else has not verbally contradicted any of the terms of this release and that the undersigned has entered into this agreement free and voluntarily without force or coercion.

Initial the following:
____ I agree that I will not consume alcohol during the boot camp. Any violation will result in twenty push-ups per occurrence.
____ I agree to not use foul language during boot camp. Any violation will result in twenty push-ups per occurrence.
____ I agree to not eat or say the words "Tim-Bits, Krispy Kreme donuts, frappucinos, french-fries, pizza, ice cream, Slurpees, chocolate bars, chips, pies, pastries, Ho-Ho's, Ding Dongs, or cupcakes" during the course of Boot Camp. Any violation will result in twenty push-ups per occurrence.
____ I agree to show up for Boot Camp every day unless it is an excused absence from my doctor or pre-approved with Boot Camp directors. Any violation will result in twenty push-ups per occurrence.
____ I understand that under no circumstances is video or photography to be taken by anyone, except Mari-Ann or The Nordic Warrior Goddess. Photos or video (i.e. May be taken during the during course of my involvement in boot camp, however "before and after" photos will not be used for any promo purposes unless authorization is granted.)
____"I understand there is NO refund policy, but I can receive a credit (for unused portion of camp) towards a future camp if I am not able to complete the one I Originally joined provided I have a Doctor's Note. If I need to transfer my boot camp credit due to medical reasons or unforeseen circumstances beyond my control, there is a $100,- Non Negotiable Transfer fee. Camp Fees cannot be used towards any other products or services provided by Mari-Ann Drevvatne or The Nordic Warrior Goddess. Payment is due in full before boot camp starts. If I am unable to attend camp due to circumstances out of my control, I am entitled to a 50% Refund of my Boot Camp Fees, as long as I notify The Nordic Warrior Goddess PRIOR to my session start date. The remaining 50% can be used as a future credit that is valid for 6 months from the start date of the boot camp that I registered for. If I notify The Nordic Warrior Goddess AFTER my session begins, I can only get a CREDIT towards a future boot camp. There is NO CASH REFUND. This is NOT Negotiable.
